I am now using \appendix instead of \chapter. 
(no error message was issued when using chapter
instead of appendix after appendices. Sorry)

1. The command for appendices i think is \appendices (not \appendicesx) which I assume was a typo.
2. The appendices numbers still appear as arabic numerals but are now prefixed with Appendix. 
(but not with Roman numbers)
3. I have the following tex warnings when I use the template I wonder if you have a clue...
